Please Note: If your child is having a problem or is not feeling well, please call the office directly, to avoid delays.
All of our forms are available online to download, fill-out and print. You may type directly into the form and print at your convenience. If you have any questions regarding which form you need, please give us a call or send us an email.
We accept most all insurance carriers, including Medicaid. It is always best that the insurer verify with their insurance carrier that we are in network prior to their visit Accepted Insurance section.
Please bring to every visit: Photo ID, Insurance Card and Shot Records (as needed). Co-pays and co insurances are expected at the time services are rendered. We accept major credit cards including Visa, MasterCard, Discover and American Express and cash.
If there have been any recent changes to your address or phone number please notify our receptionists in writing.
TIPS FOR REDUCING NEW PATIENT WAITING TIME
Prior to your appointment have all your forms filled out and if possible brought in ahead of time. Have all the appropriate documents required: Immunization records, Birth certificate, New patient packet, Insurance card, a parent/guardian DL or state ID and any important previous medical records needed such as a recent hospital visit or last physical.

For your convenience you may email the forms directly to the office to email@example.com, be sure to include the appoint time and day in your subject box and a good phone to call you in the message box in case we need to reach you.

Please arrive 15 minutes prior to your child’s scheduled appointment time. This will allow enough time to complete any necessary paperwork.
Bring your child’s insurance card. Parents/guardians should have a valid Florida State ID or Driver’s License
Parents and/or guardians must have a valid Florida State identification card or driver’s license. Please bring your child’s insurance card to every appointment. Co-pays must be paid when you arrive for your child’s visit. The forms of payment we accept are cash and most major credit cards including Visa, MasterCard, Discover and American Express.

Some questions you may want to ask:
1) Are there any co-pays associated with your visit and how much?
2) Are immunizations and physicals covered?
3) Which lab must you use and is there a co-insurance?
If there are any problems with your child’s eligibility they can be resolved prior to your visit. It is important that you verify that we accept your insurance carrier, please visit the Accepted Insurance Carriers section of our website. We accept most insurance carriers including Medicaid.
